چکیده

Introduction In a recent paper, the intracranial pressure-time burden associated with worse outcome in traumatic brain injury (TBI) patients was visualised in a color-coded plot [1]. This color-coded plot illustrates the intuitive concept that episodes of higher intracranial pressure (ICP) can only be tolerated for shorter durations: the transition curve that delineates the duration and intensity of those ICP episodes associated with worse outcome is an approximately exponential decay curve. The study was done in a large prospective multicenter European cohort of patients, including patients from before and after 2007. In 2007, the guidelines on cerebral perfusion pressure (CPP) management in severe TBI have changed [2].

Cerebral perfusion pressure management of severe diffuse head injury: effect on brain compliance and intracranial pressure.

BACKGROUND Cerebral perfusion pressure management (CPPM) is an accepted modality of treatment of severe diffuse head injury (SDHI). However, CPPM has the potential to cause transcapillary exudation in the presence of a disrupted blood brain barrier and can lead to further increase of intracranial pressure (ICP) and worsening of compliance. AIMS This study attempts to evaluate the effect of bo...

Effect of increased intracranial pressure on cerebral vasospasm in SAH.

INTRODUCTION Increased ICP is common and might precipitate cerebral vasospasm (VSP)-induced ischemic events in aneurysmal SAH (ASAH).Our objective was to determine if there is an association between increased ICP and transcranial colour coded Doppler-angiographic VSP (TCCD-A VSP) in relation to delayed neurological deficit (DND) and poor outcome. METHODS A retrospective study was undertaken i...

Effect of intracranial pressure monitoring and targeted intensive care on functional outcome after severe head injury.

OBJECTIVE : Intracranial hypertension after severe head injury is associated with case fatality, but there is no sound evidence that monitoring of intracranial pressure (ICP) and targeted management of cerebral perfusion pressure (CPP) improve outcome, despite widespread recommendation by experts in the field. The purpose was to determine the effect of ICP/CPP-targeted intensive care on functio...

Contribution of raised ICP and hypotension to CPP reduction in severe brain injury: correlation to outcome.

The aim of this study was to determine to what degree hypotension and ICP contribute to the reduction of cerebral perfusion pressure (CPP), particularly in light of the shift in emphasis to CPP management by the use of pressors. The study population consisted of severely head injured patients extracted retrospectively from the Traumatic Coma Data Bank and compared with 139 patients from the Smi...

Effective ICP reduction by decompressive craniectomy in patients with severe traumatic brain injury treated by an ICP-targeted therapy.

Severe traumatic brain injury (TBI) is one of the major causes of death in younger age groups. In Umeå, Sweden, an intracranial pressure (ICP) targeted therapy protocol, the Lund concept, has been used in treatment of severe TBI since 1994. Decompressive craniectomy is used as a protocol-guided treatment step. The primary aim of the investigation was to study the effect of craniectomy on ICP ch...
